Large grants to support the development, expansion, or maintenance of projects that promote prevention of cardiovascular disease and diabetes, engage community organizations to improve access to healthcare, and enhance health service systems of care. Funding to prevent cardiovascular disease and diabetes will support programs that implement interventions that reduce risk factors and integrate community-based outreach and education programs. Funding also supports community organizing and advocacy efforts to improve access to quality health care and capacity building to operate coordinated systems of health care. Capacity building expenses may include but are not limited to: training, placement, retention of skilled staff, and partnerships between health care providers.

Additional Eligibility Criteria:

To be eligible for funding, applicants must be:
A. Nonprofit agencies with tax-exempt status under Section 501(c)(3) of the Internal Revenue
Code and not classified as private foundations or government agencies and
B. Located within and primarily serving residents of Los Angeles County.

In addition, at least 90 percent of the population served by the organization or the proposed project must reside in the geographic areas identified by the specific ZIP codes below. Applicants must demonstrate and document client residency in the application and, if funded, in subsequent progress reports.
