Contract of services to deliver treatment, rehabilitation, and support services to persons with severe and persistent mental illness. The purpose of this funding is to provide personalized care to persons with psychiatric disabilities in order to improve their quality of life and reduce the need and frequency of state hospitalization and incarceration. Services are to be provided for persons with symptoms of severe and persistent mental illness such as: schizophrenia, other psychotic disorders (e.g., schizoaffective disorder), mood disorders such as bipolar disorders and major depression and personality disorders such as obsessive-compulsive disorders, posttraumatic stress disorders, anorexia nervosa, borderline personality disorders, and dissociative identity disorders.
