Grants to provide preventative healthcare services and education in Arizona. The purpose of the available funding is to support programs that seek to meet the needs of individuals and communities in prevention and reduction of poor health. These grants are intended to invest in the strengths and assets of individuals and communities; leverage those assets through the development of greater social capital and the protective factors that promote resiliency; and focus on prevention strategies at all levels that tap into that resiliency and result in improved health outcomes for all.

The Foundation seeks to support community partnerships that:
- Connect people
- Define issues that will build healthy communities
- Develop innovative solutions for lasting change
- Foster individual and community assets
- Recognize opportunities and take action

Must be able to demonstrate statewide impact if applicant is outside Maricopa County, as preference is given to organizations within Maricopa County.
