A grant will be awarded to an eligible organization to provide an assessment of the health care needs of Washington D.C. residents. This assessment will be used to provide hospitals and clinics with the information they require in order to provide services in alignment with community needs and eliminate health care inequity. The city-wide assessment will include an overview of the target population; indicators and benchmarks of the current health status in Washington, DC; incorporation of secondary quantitative data sources; specifically data reported within the DC-HCN web portal; incorporation of qualitative data to reflect community perspectives; stratification of data, when possible by, age, sex, gender, race, ethnicity, and ward; strengths and limitations of analysis plan; and policy recommendations.
The CHNA should be based on:
• Synthesis of secondary data sources (such as vital statistic reports, Census data, and health behavior surveys) as provided in the DC-HCN portal; and,
• Original information gathered with qualitative techniques to reflect community perspectives.
The grantee is encouraged to propose additional secondary sources of data that can inform a comprehensive assessment of community health needs and resources.
