Grants of up to $6,000 are available for Wisconsin health care facilities, including Commission on Cancer (CoC) facilities, Federally Qualified Health Centers (FQHC) and Tribal Health Clinics, for programs and projects that focus upon any of the following areas: screening for colorectal cancer for under-/uninsured, staff training and program development for survivorship care plans, and/or patient navigation.

The grant money is awarded as follows for each category:
 $1,500 – survivorship care plans
 $1,500 – patient navigation
 $3,000 – colorectal cancer screening event
