Grants of up to $50,000 to local Iowa boards of health for programs designed to address the unmet health needs of the community in one or more Counties. Questions will be accepted through November 5. Collaborative initiatives are encouraged in order to ensure that rural, micropolitan, and metropolitan counties are represented. Applicants will develop and implement sustainable initiatives which will address those areas identified in the community's health needs assessment plan.
Applicants may define the service delivery area as a single county or multiple counties, up to a maximum of 20 counties. Counties in the applicant’s service delivery area must be contiguous. If multiple applications are received that identify the same service area and the same unmet need, a maximum of one contract will be awarded to serve that service area.
IDPH encourages collaboration and seeks to ensure representation of rural, micropolitan, and metropolitan counties. IDPH intends to award a contract to a minimum of one applicant from each of the following 4 service delivery area types:
1. Multiple counties,
2. Single Rural county,
3. Single Micropolitan county, and
4. Single Metropolitan county.
