In-kind grants valued at $400 to Michigan middle and high schools to receive CPR training manikins. 5 Actar D-fib® CPR/AED Manikins are available through this grant and will be supplied with free delivery to schools within a 75-mile radius of Flushing, Michigan.

Pre-Application Information:

To be considered for 1 of 3 grants please submit a proposal, in writing to the foundation with the following information:

-School name & location, including grant writer
-Who & how many would benefit from the CPR/AED Manikins
-How often would they be used
-How many educators are CPR/AED Certified
-How do you promote Free Teen Heart Screenings in your school

All applications must be submitted on school letterhead via email or mail. Applications must be postmarked on or before Friday, December 19, 2014.

Winners will be announced Tuesday, January 6, 2015 and schools notified.

Winners must allow Thomas Smith Memorial Foundation to print and publish school name and photo.

The Foundation is requesting that each winner be working toward (or already received) the MI HeartSafe School Certification (more information may be found in the attachments below).
