Grants to Montgomery County, Maryland healthcare nonprofit organizations for capacity-building initiatives. Applicants must attend a training session prior to applying. Funding is intended to improve the quality and accessibility of healthcare; expand the availability of healthcare; build capacity in the healthcare network; and grow the healthcare workforce. The purpose of the grant is to start or grow well-conceived programs and organizations.

FY20 Capacity Building Grant priorities aim to promote sustainable, culturally competent, integrated healthcare access throughout the lifespan through: 

A.  Behavioral Health Across the Lifespan:

  • Promote and/or implement trauma-informed practices;
  • Treat and prevent domestic violence and sexual assault;
  • Support integrated solutions for the treatment and prevention of substance abuse and opioids;
  • Support suicide prevention and resiliency building; and
  • Develop interdisciplinary patient care models supporting integrated behavioral health and coordinated continuity of care services for Montgomery County residents (e.g. in schools, school based health center, homeless services, safety-net clinics, etc.).

B.  Access to Quality and Comprehensive Health and Wellness Services:

  • Provide integrated, sustainable safety-net services across the lifespan to infants, children, adolescents, adults, and seniors (e.g. family practices, dental services, vision exams and eye-glasses, chronic disease and pain management, tele-medicine, prevention services, maternal health and wellness, geriatric services, etc.);
  • Ensure that ALL safety-net health clinics serving Montgomery County’s vulnerable and diverse populations offer the same high-quality, culturally competent core ambulatory care services and meet patient quality of care and outcome benchmarks; and
  • Develop multi-sector approaches to improve health and wellness by addressing the social determinants of health-adverse childhood experiences, race equity, food security, affordable housing, employment, education, quality childcare, environment, and access to healthcare and wellness services.

C.  Sustainable Business Models and Integrated Service Delivery Systems to Stimulate Systemic, Positive Change in the Health and Wellness Sector:

  • Establish sustainable business models around the provision of essential health and wellness services (e.g. diversified patient payee mix, Medicare, Medicaid, fee for service, family practice models, pay for performance models, etc.); and
  • Develop place-based initiatives to address multi-sector, multi-generational, and/or complex health and wellness issues (e.g. aging in place, multi-generational poverty, health disparities, race equity, subsidized housing, food insecurity, etc.).

D.  Develop a Culturally Competent and Highly Skilled Health Sector Workforce:

  • Build educational and workforce pipelines for culturally competent, highly skilled health sector workers to meet the current and future health and wellness needs of Montgomery County residents;
  • Design innovative apprenticeships/internships for emerging health professionals to both train and expand the capacity to deliver health and wellness services to Montgomery County’s vulnerable residents; and
  • Support inter-sector training, certifications, and/or help with career development and pathways within the health and wellness sector.

Additional Eligibility Criteria:

The Healthcare Initiative Foundation considers grants to 501(c)(3) healthcare nonprofits that are based in Montgomery County, Maryland, or to nonprofits that have county-based health programs or activities.

The Funding Source Will Not:
-Give to endowment funds or annual campaigns;
-Fund lobbying or political activities;
-Make grants to individuals;
-Provide funds to private foundations unless for a particular grant purpose;
-Fund a non-profit system or organization, including its affiliate(s) that owns or operates an acute care hospital in Montgomery County, Maryland;
-Provide funds to cover brick and mortar capital expenses;
-Replace government funding; or
-Fund religious activities, although secular health programs provided by a religious institution or its affiliate(s) may qualify.

Pre-proposal Conference:

All organizations interested in applying for grant funds from HIF in FY20 must attend training. The next grantee training will be Friday, November 1st. The focus will be Technical Support training for FY20 Capacity Building applications. The training will cover: updates to the FY20 application, HIF’s budget template guidelines, and outcomes vs impacts requested in our application.

Pre-Application Information:

Capacity Building applications will be available online Friday, November 1, 2019 and due Friday, December 6, 2019, by 5 pm. All applications are submitted online.

During grant application period, organizations that have not had a previous site visit should contact Jess Fuchs to schedule a site visit. If you have had a site visit and would like another visit, this is welcomed during this period as well.
