Request a Grant Writer

Grants of up to $2,500 to Wisconsin government agencies, nonprofits, individual health practitioners, and ad hoc groups to advance the health and wellness of local communities in eligible regions. Eligible program areas include health, community betterment, education, disease screening, fitness and wellness assessment or improvement, and research.

Additional project guidelines:

  • Services must be delivered to residents of the area served by Sauk Prairie Healthcare including Black Earth, Lodi, Plain, Prairie du Sac, Sauk City, Spring Green and others.
  • Consideration will be given to programs that are finite in scope and duration and start-up projects that show uniqueness, creativity and impact.
  • The Foundation will consider matching funds for programs partially funded through other sources.
  • Consideration will be given based on the number of people served

A few of the initiatives that have been supported include hiking programs, senior fitness programs, CPR training mannequins, school and community gardens, and local food pantries.

The maximum individual grant amount is $2,500.

Term of Contract:

Funds will be available after January 1, 2020. Programs must commence during 2020.

Additional Eligibility Criteria:

Services must be delivered to residents of the area served by Sauk Prairie Healthcare, including Black Earth, Lodi, Plain, Prairie du Sac, Sauk City, Spring Green, and others.

The following are eligible to apply:

- Practitioners in all fields of health (excluding Sauk Prairie Healthcare)
- Educators, elementary and secondary schools and school districts
- Community employers, civic organizations and service clubs
- Government agencies
- Libraries, museums, and historical societies
- Ad hoc groups assembled for the purpose of undertaking a specific project.

Collaborative proposals are welcomed.

In general, the Foundation will not consider funding for:

- Direct support of an individual’s care
- Capital projects (“bricks and mortar”)
- Direct or indirect support of lobbying or other political activity
- Programs whose primary or sole purpose is marketing
- Ongoing general operating expenses or existing deficits
- Registration fees for individuals or teams for fundraising events
- Program ads, door prizes, raffle tickets, dinner tables, golf outings or sponsorships of organized sports teams or similar activities
- Registration or travel fees for conferences
- Previously funded organizations who did not submit grant reports

Pre-Application Information:

Submission deadline is 4:30 pm on Tuesday, October 1, 2019. The intent is to send grant notification letters by December 31, 2019.

The Foundation prefers that you scan and email your proposal. Hand-delivered, mailed, or faxed applications postmarked or time stamped before the deadline are also accepted. Email your scanned grant proposal using the subject line “Health Life Grant Submission.”
