Grant Description
Grants of up to $2,500 to Wisconsin nonprofit organizations, schools and school districts, government agencies, museums, historical societies, libraries, ad hoc groups, and individuals in eligible locations for community health programs. Services must benefit residents of the area served by Sauk Prairie Healthcare, including Black Earth, Lodi, Mazomanie, Plain, Prairie du Sac, Sauk City, Spring Green, and others.

Healthy Life Grants fund a wide variety of wellness projects, including hiking and biking trails, literacy programs, senior fitness equipment, CPR training mannequins, defibrillator purchases, EMT and lifeguard training, school and community gardens, and local food pantries.

The purpose of the Healthy Life Grant program is to advance the health of our communities by funding new or expanded screening, educational, preventive and wellness projects or by funding research which can be applied to such projects.

Eligibility Guidelines:

  • Eligible programs will fall into the broad definitions of: health, community betterment, education, disease screening, fitness/wellness assessment and/or improvement, or research.
  • Consideration will be given to programs that are finite in scope and duration and start-up projects that show uniqueness, creativity and impact.
  • Consideration will be given based on the number of people served.

Additional Eligibility Criteria
Eligible to Apply:
- Practitioners in all fields of health (excluding Sauk Prairie Healthcare)
- Educators, elementary and secondary schools and school districts
- Community employers, civic organizations and service clubs
- Government agencies
- Libraries, museums and historical societies
- Ad hoc groups assembled for the purpose of undertaking a specific project. (Collaborative proposals are welcomed.)

Ineligible
Limitations:
In general, the Foundation will not consider funding for:
- Previously funded organizations who did not submit required grant reports
- Direct support of an individual’s care
- Capital projects (“bricks and mortar”)
- Direct or indirect support of lobbying or other political activity
- Programs whose primary or sole purpose is marketing
- Ongoing general operating expenses or existing deficits
- Registration fees for individuals or teams for fundraising events
- Program ads, door prizes, raffle tickets, dinner tables, golf outings or sponsorships of organized sports teams or similar activities
- Registration or travel fees for conferences

Pre-Application Information
Submission deadline is 4:30 pm on Friday, September 29, 2023.

The intent is to send grant notification letters by December 31, 2023.

Funds will be available after January 1, 2024.

The funding source will consider matching funds for programs partially funded through other sources.

The funding source prefers that you scan and email your proposal. They will also accept hand-delivered, mailed or faxed applications postmarked or time stamped before the deadline.
