Grants to Rhode Island nonprofit and faith-based organizations for place-based collaborative efforts to improve the health of communities with the highest rates of chronic disease, illness, or other adverse health outcomes. Funding is intended for projects that bring together clinical organizations, community-based agencies, and community members to implement a shared vision and action plan to address social determinants of health and improve the health of local communities.

Proposals may be for projects one to three years in duration, and up to $200,000 per year.

Proposals will be evaluated on the strength of their alignment with the following elements:

1. Collaboration for impact
2. Community engagement
3. Needs assessment
4. Action plan
5. Evaluation plan
6. Implementation capacity
7. Financial leverage

Up to $200,000 per year

Term of Contract:

Proposals may be for one to three years in duration.

Funding and reporting periods will be structured as follows:
- Year 1 (Oct 2018 – Oct 2019)
- Year 2 (Nov 2019 – Nov 2020)
- Year 3 (Dec 2020 – Dec 2021)

Additional Eligibility Criteria:

- Nonprofit, 501(c) organizations located in the state of Rhode Island are eligible to apply.

- The FHRI does not fund individuals, scholarships or research, capital or endowment efforts, or fundraising events.

- Grants may support faith-based organizations for secular programs or projects.

- Research as part of a program evaluation is permitted, however, Institutional Review Board (IRB) approval or exemption must be obtained where human subjects are involved.

- The Rhode Island Foundation believes that our community can best be served by charitable organizations that both reflect and serve the diversity of our community. Grants are not awarded to applicants or for programs that have a policy that discriminates against any person or group in any way that is either unlawful or inconsistent with the mission or values of the Foundation.

Pre-Application Information:

The application process is comprised of three steps: 1) Stage 1 application; 2) Stage 2 application by invitation; and 3) site visit. The entire application and reporting process is to be completed online.

The deadline to submit a Stage 1 application is 4:00 PM on April 2, 2018. All applicants will be notified of their standing by the end of April 2018.

Please note: The Foundation seeks projects that leverage support from other funding sources.
