Grant Description
Grants of up to $30,000 to Massachusetts nonprofit and faith-based organizations, schools, and government agencies to improve health in eligible communities. Funding is available for projects and programs that promote health equity, improve the mental and physical health of local residents, and address unmet public health needs. Applicants must serve Barnstable County.

CCHC Community Benefits is pleased to announce the FY2022 Annual Strategic Grant Request for Proposals (RFP). Organizations are invited to submit proposals for programs or projects that:

  • Improve the physical and/or mental health of Cape Codders
  • Address unmet public health needs
  • Promote health equity

Projects that also address one or more of the following Social Determinates of Health will be given extra consideration: Transportation, Housing, Workforce Development.

All proposals funded through the Annual Strategic Grant RFP process must tie back to the 2020- 2022 Cape Cod Hospital and Falmouth Hospital Community Health Needs Assessment Report and Strategic Implementation Plan (CHNA). The five priority areas identified in the 2020-2022 CHNA are:

  1. Physical Health Conditions
  2. Behavioral Health
  3. Transportation
  4. Housing
  5. Workforce Development

Additional Eligibility Criteria
Types of Organizations That May Apply:
- 501(c)(3) public charities
- Organizations serving a charitable purpose that have a fiscal sponsor that is a 501(c)(3) public charity
- Faith-based organizations. CCHC will consider proposals if: (1) services benefit all, regardless of religion; (2) services provided are not religious in nature; and (3) there is no proselytizing associated with the service.
- Civic organizations. Proposals must be project-based and of a charitable nature; the organization must have a fiscal sponsor that is a 501(c)(3) nonprofit organization.
- Public and private schools. CCHC will only fund projects that exceed the school’s budget. Applicant must have a letter of support from the fiscal head of the school.
- Other government agencies with project-based proposals. Applicants must have a letter of support from the fiscal head of the agency.

CCHC Community Benefits will not fund the following requests through this process:
- Programs outside of stated priorities
- Programs serving areas outside Barnstable County
- Duplicative programs in the same service area
- Political or fundraising campaigns
- Construction or renovation activities, leased property or property acquisitions
- Event expenses or incentives including food, beverages, gifts, or tokens
- For-profit ventures

Pre-Proposal Conference
Community Benefits will host two grant information sessions at the Cape Cod Healthcare Foundation, 32 Main St., Hyannis:
- Monday, Aug. 15, 2-4 p.m.
- Tuesday, Aug. 16, 10 a.m.-noon

These sessions will provide an overview of Cape Cod Healthcare’s Annual Strategic Grant process and are intended for community members who are new to Cape Cod Healthcare’s grant-making process. All are welcome and registration is required by emailing staff. If you cannot make one of these information sessions, please feel free to reach out to staff to make an appointment for assistance. Note: You do not have to attend a session to submit a grant application.

- RFP Release: August 2022
- Deadline for Proposal Submission: September 15, 2022
- Notification of Awards: December 2022
- All invoices must be received by: September 1, 2023
- Annual Summary & Outcomes Report Due: November 30, 2023

Grant recipients will invoice for grant payments. All invoices must be received by September 1, 2023.
