Grants starting at $15,000 to Minnesota nonprofit and for-profit organizations providing home care, housing, and adult day care services to the elderly. Funding is intended to offer educational scholarships and training to the caregiving staff of qualified organizations. Eligible courses of study for scholarship recipients include workshops and degree programs directly related to the delivery of patient or client care in home and community-based services settings.

Grant outcomes will include:
- Increase the HCBS workforce
- Recruit individuals to work in the HCBS field, particularly with the aging population
- Retain professionals in the HCBS field

All expenses must be directly related to the employee’s coursework or training, and may include:
- Employee tuition. Tuition may be paid directly to the educational institution or reimbursed to the employee upon completion of the course
- Course-related textbooks or fees
- Childcare expenses while the employee is attending classes
- Mileage reimbursement
- Training consultant fees

Additional Eligibility Criteria:

Not-for-profit and for-profit organizations that meet all of the following criteria are eligible to apply for a HCBS Employee Scholarship grant. An eligible applicant organization or facility:
- Is located in Minnesota;
- Primarily provides services in home and community-based settings to individuals who are 65 years of age or older;
-Offers an in-house employee scholarship program to its staff, or is in the process of developing a scholarship program.

Additionally, organizations must be one of the following licensed providers:
- Housing with Services establishment as defined in MN Statute 144D.01
- Adult Day Care facility as defined in MN Statute 245A.02
- Home Care Services providers as defined in MN Statute 144A.43, subd. 3

Ineligible expenses include but are not limited to:
- Administrative expenses to manage the grant
- Employee salary, fringe benefit or stipends while the employee is attending school
- Out-of-state travel or lodging
- Supplies or equipment that are not directly related to classes/coursework of scholarship recipients
- Capital improvements
- Fundraising
- Taxes, except sales tax on goods and services
- Lobbyists, political contributions
- Bad debts, late payment feeds, finance charges, or contingency funds

Pre-Application Information:

Please submit questions no later than 4:00 PM Central Time, on April 19, 2019. If MDH determines questions and answers should be posted on the MDH website, they will be available at:
https://www.health.state.mn.us/facilities/ruralhealth/funding/index.html#hcbs.

All applications must be received by MDH no later than 4:00 PM Central Time, on April 19 2019. If applications are mailed, they must be received by MDH by the deadline.

MDH will notify successful applicants of funding decisions in writing, sending a letter to the primary contact listed on the grant application. Letters will be sent on or around May 17, 2019.

Per State Policy on Grant Payments, reimbursement is the method for making grant payments.
