Grants ranging from $5,000 to $50,000 to Rochester area, New York nonprofits, religious and educational organizations, and government agencies for programs to benefit residents' health. Funds may be used for activities such as start-up costs for new programs, bridge funding, short-term programs, pilot projects, equipment and facility repair, and staff training for specific skill development.

Eligible Counties include Genesee, Livingston, Monroe, Ontario, Orleans, Seneca, Wayne, Wyoming, and Yates. Preference will be given to smaller grants submitted by smaller organizations with an annual budget of $5 million or less that did not receive a Community Health Grant in 2016.

The mission of the Greater Rochester Health Foundation is: “to improve the health status of residents of the Greater Rochester community, including people whose unique healthcare needs have not been met because of race, ethnicity or income.”

Grants range from $5,000 to $50,000. The average grant award amount for the first six cycles of Community Health Grants was $12,000. Grants at the $50,000 maximum level will be the rare exception.

Grants are to be used during the 12 months following the 2017 grant award.

The Health Foundation will consider proposals from nonprofit organizations, health, religious or educational organizations and government departments that provide health services or health-benefiting activities in Genesee, Livingston, Monroe, Ontario, Orleans, Seneca, Wayne, Wyoming, and Yates counties.

Indirect costs, education programs without hands-on learning, general operating costs including existing staffing, endowment campaigns, fundraising events, political lobbying activities, capital campaigns, and differences between third party payments and actual costs will not be considered.

Application workshops are available for each grant cycle. These workshops are a hands-on opportunity for small organizations with limited grant writing support or resources to improve their ability to complete a Community Health Grant application.

Proposal Deadlines:

- Cycle 1-Applications open: Feb 1, 2017. Proposal deadline: March 15, 2017 at 5:00 PM
- Cycle 2-Applications open: June 8, 2017. Proposal deadline: July 21, 2017 at 5:00 PM

You may apply more than once this year but only one Community Health Grant will be awarded to your organization in 2017.

Community Health Grants have a rapid award process. Applicants will be notified of a decision within one month of the application deadline. If a Community Health Grant is approved, funds will become available upon finalization of your grant agreement.
