Grants ranging from $5,000 to $30,000 to Colorado health systems to enhance preventive health services available to underserved residents. This program supports the implementation of evidence-based interventions for targeted disease areas. Grantees will address designated public health performance criteria.

This project engages with health systems to assess current performance on select health measures, increase capacity to monitor performance, identify public health focus areas, and implement evidence-based interventions (EBIs) proven to improve health outcomes.

This project serves to solicit responses for improved performance within health systems that serve underserved Coloradans on targeted performance measures.

Estimated Size of Grant:

The estimated project budget awards can range from $5,000 to $30,000 per award per year. Additional funds may be made available over time and may result in increased or additional awards.

Term of Contract:

Funded applicants will receive a contract beginning approximately two months after the awarding period deadline and ending June 29, 2020. All contracts are contingent upon funded applicant performance and availability of funds. Contract renewal on June 30 of each year following the applicants initial award through 2019 is subject to a renegotiation of project work plan and budget through June 29, 2020.

Additional Eligibility Criteria:

To be eligible, an applicant must be 1) a health system providing preventive services in the State of Colorado or 2) a support, network or member organization for one or more health systems providing preventive services in the State of Colorado.

To be eligible, health systems listed within the application must have completed eligibility activities as outlined in the CQI Guide including completing Phase 1 through the American Cancer Society or other entity contracted by CDPHE, completing Phase 2 through CDPHE, meeting criteria for CQI certification, and receiving a letter of certification from CDPHE’s CQI project recommending the health system respond to the Clinic Quality Improvement for Population Health Implementation Grant.

Pre-Application Information:

Approximately $350,000 of state and/or federal funds are currently available for this RFA to be awarded to multiple sub-recipients during the State fiscal 2016 year. This annual budget dollar amount is anticipated for each State fiscal year up to the fiscal year ending June 29, 2020.

Questions from applicants will be accepted until January 15, 2019. Answers will be posted monthly through January 15, 2019.
