Grants to Cuyahoga County, Ohio nonprofits and agencies for programs that emphasize community health and preventing obesity, physical improvements in neighborhoods, and family development and well-being. Applicants must meet with program staff in advance of submitting an application.

Program Areas:

Healthy People: Improve community health by promoting patient-centered care and reducing obesity. Goals are:
- Residents have holistic access to and support from the healthcare sector, and they fully participate in a patient-centered model of care
- Residents can access healthier food choices, engage in physical activities, leverage social support and work to reduce obesity

A Strong Neighborhood: Improve social conditions and improve the physical environments in Mt. Pleasant, Woodland Hills, and Buckeye. Goals are:
- Residents and neighborhood organizations collaborate to build improved physical environments
- Residents and neighborhood organizations proactively unite and contribute to improved social conditions

Resilient Families: Ensure that community resources meet families’ needs and empower parents to implement their vision for the healthy development and well-being of their families. Goals are:
- Families have access to concrete, dependable resources and support in times of need
- Parents feel empowered to improve the health and well-being of their families

Saint Luke’s Foundation provides support through the following grant types:

Operating Support - Unrestricted funding in support of an organization’s programming and infrastructure
Program Support - Targeted support for a program or initiative
Discretionary - Funding of up to $20,000 for short-term, emergency needs
Capacity Building - Support to improve organizational effectiveness

Additional Eligibility Criteria:

Applicants must be either an organization designated as a public charity under Section 501(c)(3) of the Internal Revenue Code, with a non-private foundation status with the IRS, or a governmental unit or agency. The Foundation provides funds to organizations supporting Cuyahoga County’s underserved communities as operational, project, capital, or capacity building grants. The Foundation does not fund individuals, religious organizations for religious or evangelical purposes, projects outside Cuyahoga County that do not directly benefit Cuyahoga County residents, fundraising events, endowment funds, debt retirement, lobbying, and private foundations or public charities with a 509(a)(3), Type III., Non-Functionality Integrated designation with the IRS.

Discretionary grants of $20,000 or less are accepted and approved on a rolling basis. Please contact the appropriate Senior Program Officer prior to submitting an application. Applicants will be notified of a decision within four weeks of submission.
