Western Maine Health - Stephens Community Healthcare Foundation (SCHCF)
Grants to Western Maine nonprofit organizations to improve the accessibility and affordability of health services. Funding may be requested for projects, programs, services, capital contributions, and matching pledge contributions. The Foundation may support the purchase of state-of-the-art medical equipment, community education initiatives or promoting health programs, and other activities that ensure a healthy future for the people of Western Maine.
Primary consideration will be given to requests that meet the following criteria:
-Demonstrate a clear relationship to the Foundation’s mission
-Demonstrate a direct relationship and benefit to health care in the immediate service area of Stephens Memorial Hospital
-Are innovative in approach or outcome
-Document receipt of matching gifts or additional funding from other sources.
-Demonstrate benefit to the community for the resources invested, e.g., resources used compared
with numbers of individuals reached or degree of improvement produced
-Promote collaboration among organizations without duplicating services/activities
-Enhance or improve project self-sufficiency

The Foundation generally does not fund projects or programs that require perpetual funding, normally more than three years.
Funding awards are limited to nonprofit 501(c)(3) organizations based in or benefiting healthcare initiatives in the Stephens Memorial Hospital service area.
Individuals wishing to apply for funds must do so under the sponsorship of the 501(c)(3) organization.
Funding of programs that require start-up operations will be based upon an evaluation of compliance to the mission, funding purposes, and benefit to the involved constituents and will be supported by an estimated budget, projected funding period, anticipated outcomes and measurable operational benchmarks.
Non-Eligible Funding Requests:
-Lack sponsorship by or benefit to a nonprofit organization outside of the Stephens Memorial Hospital service area.
-Are designed to supplement or replace operating deficits
-Funds to cover Administration, salaries, marketing, buildings or operational deficits
-Are normally the responsibility of a public or governmental agency
-Require perpetual funding, normally more than three years
The application deadline is Monday, December 31, 2016 for consideration in January. Funding decisions will be announced by February 1, 2017.
