Quality Health Foundation (QHF)

Grants of up to $50,000 to Maryland and Washington, DC nonprofit organizations, including faith-based organizations; hospitals, providers, or clinics; educational institutions; and community-based service organizations for programs and projects that aim to improve the health of local residents.
This RFP is also specifically soliciting proposals for a program particular to pulmonary and/or lung disease, i.e., but not limited to smoking cessation, emphysema, and chronic bronchitis. A high priority will be given to applications that reach underserved populations.
Examples of items that have been funded include: salary for medication coordinator; development, printing and distribution of educational materials; and arranging transportation to secure medical services.
The Foundation's focus is on supporting programs that demonstrate improved health care quality with clear goals and measurable outcomes. Priority areas include programs that:
- Improve access to health care services to the unserved and under-served;
- Improve understanding of health issues; and
- Demonstrate improvement through measurable outcomes of stated goals.
Types of programs QHF might fund include those that:.
- Improve healthy lifestyles and quality of life
- Increase access to culturally and linguistically appropriate care
- Implement consumer centered care and decision-making
- Improve dissemination of education and health related information
- Improve patient safety

These awards may be up to $50,000.
Customarily, grant awards are for one year with the potential for additional funding in subsequent years.
The next grant year will begin on July 1, 2017.
Applicants must be from Maryland or the District of Columbia.
All organizations with current 501(c)3 status focused on improving the health of individuals through measurable outcome goals at the community level are encouraged to apply. Examples of organizations that may apply for funding are:
- Faith-based organizations
- Hospitals, providers, or clinics
- Educational institutions
- Community service organizations
QHF will not fund:
- Reimbursable direct patient care services
- Facility construction/remodeling of facilities
- Fundraising activities
Salaries for administrators are not usually funded; and ongoing operational expenses such as utility bills and rent are not funded.
To be considered, all organizations must complete the attached Request for Proposal form. The deadline for submission of the completed form, including the required attachments, is 5:00 p.m. EST on January 17, 2018.
The final decisions are made by the QHF Board of Directors. All eligible proposals will be reviewed and grantees will be selected in May/June 2018 and notified of the disposition of their request by mid-June 2018.
