Grants ranging from $15,000 to $80,000 to Chicago Metropolitan Area, Illinois nonprofit organizations to benefit medically underserved populations. Applicants must submit an LOI prior to submitting a full application. Applicants seeking multi-year support must contact program staff prior to applying. Funding may be requested for capital projects, operating support, start-up support, and program support.
Programs must benefit residents of Cook, Lake, McHenry, DuPage, Kane and Will Counties, with a focus on Chicago.
VNA Foundation funds must be used only for the medically underserved. This is accomplished through grants to agencies that work exclusively with this population, or by funding specific programs that target the under- and uninsured.
All grants must support home- or community-based health care and health services.
In deference to the Foundation’s roots as the Visiting Nurse Association of Chicago, priority consideration will be given to programs in which care is provided by nurses.
The Foundation will consider requests in the following areas:
-Home health care services
-Community and school-based services
-Primary care and chronic disease management

Each request is considered on an individual basis, but grants generally range from $15,000 to $80,000.
The VNA Foundation does make multi-year grants, but infrequently.
All grants are made to qualified nonprofit agencies exempt from federal income tax under section 501(c)(3), and designated as public charities under section 509(a)(1) or 509(a)(2), of the Internal Revenue Code.
VNA Foundation does not provide grants to section 509(a)(3) “supporting organizations.”

Based upon a review of the Letter of Intent, you will either be asked to submit online a complete copy of a full proposal or be advised to look elsewhere for funding.
Grantees interested in renewal funding, and those with multiyear awards, should contact their Program Officer to discuss acceptability and timing of a request.
