Grants to Ohio nonprofit organizations government agencies, and public schools for initiatives to address community health issues in eligible locations. Priority areas include responses to the Coronavirus (COVID-19) health crisis and a range of other health and well-being topics. Funding is intended to benefit residents of Trumbull County.

Reflecting its purpose of promoting the health and well-being of the residents of Trumbull County, Trumbull Memorial Health Foundation’s 2021 grantmaking will prioritize grants that align with one or more of the below areas:

  1. Immediate responses that address obstacles to health and wellbeing for residents due to the COVID-19 pandemic
  2. Innovative projects that address the current priorities identified by the 2020-2022 Trumbull County Combined Health District Community Health Improvement Plan
    • Mental Health and Addiction: initiatives that increase mental health and decrease substance abuse
    • Chronic Disease: initiatives that decrease chronic disease and obesity
    • Maternal and Infant Health: initiatives that improve maternal and infant health outcomes
  3. Organizations and programs that address areas identified by issue-specific funds held by Trumbull Memorial Health Foundation:
    • Needs of cancer patients and/or their families
    • Hospice services, care, or education
    • Diabetes care, including prevention programs and education
    • Orthopedic care and/or support for patients including education

Additional Eligibility Criteria:

Organizations eligible to receive grants from the Community Foundation of the Mahoning Valley are those determined by the IRS to be public charities. This encompasses most charitable, scientific, social service, educational, and religious organizations described in section 501(c)(3) of the tax code, as well as government entities and public schools.

Organizations that are not public charities may apply through a fiscal sponsor.

The Trumbull Memorial Health Foundation values the efforts of the Healthy Community Partnership, a regional cross-sector collaborative working to address several of the CHIP’s priorities and strategies. Priority is given to grant seekers who are connected to the Healthy Community Partnership and aligned with the shared vision and goals. If your organization is not currently active in the Healthy Community Partnership, please contact Casey Krell.

Requests from individuals and fro activities to influence election campaigns are not eligible while requests for retroactive funding for projects that have been completed or will be completed prior to the grant award date are typically not supported.

Pre-Application Information:

2021 Application Deadlines: April 19, July 1, October 1. Applications are due by 11:59 p.m.

While the Foundation does not prohibit reapplying in a set period of time, multiple, frequent requests from an organization are not encouraged.
