Grants to Ohio nonprofit organizations, schools, churches, and government agencies for programs that address community health issues in eligible regions. Applicants must submit a letter of inquiry prior to submitting an application. Priorities for funding include programs that address chronic disease, mental health, addiction, and maternal and infant health. Programs must benefit residents of Trumbull County.

Reflecting its purpose of promoting the health and well-being of the residents of Trumbull County, the Trumbull Memorial Health Foundation requests proposals for innovative projects that address the current priorities identified by the 2020-2022 Trumbull County Combined Health District Community Health Improvement Plan.

Trumbull County Combined Health District Community Health Improvement Plan priorities:

  1. Mental health and addiction: Initiatives that increase mental health and decrease substance abuse
  2. Chronic Disease: Initiatives that decrease chronic disease and obesity
  3. Maternal and Infant Health: Initiatives that improve maternal and infant health outcomes

In addition, the Trumbull Memorial Health Foundation gives priority to projects that:

  • Address underlying social and economic challenges contributing to poor health outcomes in the communities served, such as poverty, lack of access to healthy foods and absence of parks/recreational centers
  • Extend past individual behavior or program related services by modifying policies, systems and environments to help make healthy options practical and readily available
  • Demonstrate collaboration among sectors, including public health, government, education, healthcare, etc.
  • Prioritize engagement of residents from the communities served
  • Adhere to existing evidence-based best practices or promising practices
  • Leverage additional funding as well as community assets
  • Provide an established plan for evaluation including measurable outcomes

Additional Eligibility Criteria:

Eligible applicants must be tax-exempt organizations under Internal Revenue Service 501(c)(3) or operate under the fiscal sponsorship of a tax-exempt non-profit, including schools, churches, and public organizations. Eligible organizations must provide services which contribute to the health and wellness of the residents of Trumbull County.

Pre-Application Information:

Interested applicants must submit a letter of inquiry form (LOI) on or before January 17, 2020. Please do not submit a full proposal unless invited to do so. If the LOI is accepted, an application form will be made available.

Those invited to submit a full proposal will be notified in early March 2020. Completed applications with supporting documents will then be submitted to the Trumbull Memorial Health Foundation on or before April 10, 2020. Final grant decisions will be announced in June 2020.
