Grants ranging from $15,000 to $50,000 to Rhode Island nonprofit organizations to prevent and mitigate adult and childhood diabetes. Programs may involve education, nutrition, and training, as well as exercise and treatment programs for diabetic and pre-diabetic individuals.

Treatment programs may be carried out in conjunction with healthy lifestyle options. Particular emphasis is placed on disadvantaged populations and measurable outcomes with the broadest impact.

Proposals must address the scope of the program; number of patients it will help, how access to effective healthy food choices and nutritional guidance will be achieved in a timely manner; how patient progress will be monitored and tracked; and how outcomes will be measured.

Preference given to new proposals addressing critical gaps in unmet need as described above.

$50,000

Funding may be awarded to one or more agencies.

$15,000 - $50,000

Applicants must be a 501(c)3 Rhode Island nonprofit in good financial standing; grants will be awarded to organizations, not individuals.

The deadline for submission is September 15, 2017; applicants being considered may be contacted to arrange a site visit to complete the approval process.
