Grants to Kentucky and Indiana nonprofit organizations and governmental and public entities, including schools, for health initiatives that benefit eligible communities and address local community health needs. Funding is intended to support activities that provide measurable improvement in health status, access or use of healthcare resources and that serve individuals in the Owensboro Health service area.
The Owensboro Health service area includes the following counties: Breckenridge, Butler, Daviess, Grayson, Hancock, Henderson, Hopkins, Muhlenberg, McLean, Ohio, Union and Webster in Kentucky and Perry and Spencer Counties in Indiana.
Grants are also awarded to nonprofit organizations for specific programs and activities not directly related to healthcare but addressing root causes of health problems that impact the health of the community. Such grantees may include economic, educational, civic, arts, athletic, and cultural organizations. To qualify, the proposal must identify how the activity addresses root causes of identified health needs.
Additional factors that will warrant priority consideration:
- Multi-partner proposals focused on a priority health need.
- Partnerships with local health councils.
- Collaboration with other organizations in all aspects of program planning.
- Proposals utilizing evidence-based or promising practices.
All programs, projects and activities must include objectives that are SMART: Specific, Measureable, Attainable, Realistic and Time-bound.
Applications must demonstrate a commitment to provide services regardless of race, religion, sex, age, disability or national origin.

Support will generally not exceed one year.
Eligible applicants are designated 501(c) 3 nonprofit organizations, governmental, or public entities who are working to address identified community health priorities.
Eligible organizations must serve one or more of the following counties: Breckenridge, Butler, Daviess, Grayson, Hancock, Henderson, Hopkins, Muhlenberg, McLean, Ohio, Union and Webster in Kentucky and Perry and Spencer Counties in Indiana.
Owensboro Health will only consider applications by schools and school-affiliated groups within the Owensboro Health service area which are authorized by and coordinated through the school’s central office and that which supports the respective school district’s health and wellness plan. Evidence of that plan will be required.
Applicants must provide evidence of their organization’s 100 percent tobacco-free policy.
Partisan political groups or activities, endowments, memorials, individuals, field trips, or tours are not eligible to receive funding.
Applications requesting assistance for salaries, scholarships or memberships will only be considered as part of a larger, comprehensive program or project.
It is recommended that any organization considering a grant application attend a grant information session.
