Grants up to $3,000 to Western Montana organizations that provide palliative and hospice care services. Priority will be given to organizations with nonprofit status, and those organizations serving individuals in Lake, Missoula, Mineral, and Sanders Counties.
Funding is available for activities, projects, and programs serving patients in the last six months of life, as well as to family members after the patient’s passing. The Project/Program Support Grant award was implemented to fund organizations so that they can continue palliative care programs, activities and/or projects and/or implement new ones.
Activities may include:
-Bereavement/Grief Support Groups
-End of Life Palliative Care Programs (such as pet therapy, music therapy, hydro therapy, art classes, aromatherapy, etc.)
-Spiritual Care Programs
-Hospice/Palliative Care Volunteer Training Programs

Maximum Awarded per Organization: $3,000.
Additional funding above $3,000.00 per organization may be awarded should additional funding become available and only if approved by the Board of Directors.
Eligible organizations include those in Western Montana who provide palliative and/or hospice care programs/projects/activities.
Priority will be given to organizations that provide services to individuals within Missoula, Lake, Sanders, and Mineral Counties.
Priority will be given to nonprofit organizations.
-Program/Project Support Grants do not cover cost to run a hospice and/or palliative care program.
-Projects/Programs/Activities must benefit community members within western Montana.
-Organizations must take the Eligibility Quiz if they wish to apply for a 2018 Program/Project Support Grant.
-Only online applications will be accepted.
-All supporting attachments must be submitted with the grant application.
-Incomplete applications will not be considered for funding.
-Application must be completed and received by HCF during the specified time frame to be considered for funding.
-Applications for 2018 Grants are accepted starting Wednesday, November 1, 2017 and must be received by 3:00 pm MT Monday, January 15, 2018. Applications received outside of this time frame will not be reviewed.
-Awardees will be notified no later than Tuesday, April 13, 2018.
