Grants of up to $1,000 to Western Montana organizations that provide palliative and hospice care services. Priority will be given to organizations with nonprofit status, and those organizations serving individuals in Lake, Missoula, Mineral, and Sanders Counties. Funding is intended for activities, projects, and programs serving patients in the last six months of life, as well as to family members after the patient’s passing.

Project/Program Support Grants were implemented to fund organizations so that they can continue palliative care programs, activities and/or projects and/or implement new ones.

Activities may include: 

  • Bereavement/Grief Support Groups
  • Palliative Care Programs (i.e. pet therapy, music therapy, hydrotherapy, art classes, aromatherapy, etc.)
  • Spiritual Care Programs

Number of Grants:

HCF anticipates funding five (5) Program/Project Support Grants for 2020.

Estimated Size of Grant:

Maximum Awarded per Organization: $1,000.

Additional Grants or Grants of more than $1,000 may be awarded should additional funding become available and only if approved by the Board of Directors.

Term of Contract:

If full grant amount is not expended within 12-months from award date, the organization may apply for a 12-month No-Cost Extension in order to fully utilize the award. Organizations may apply for two no-cost extensions for a total of 36 months’ time to expend grant funds.

Additional Eligibility Criteria:

Eligibility:
- Eligible organizations include those in Western Montana who provide hospice, palliative care, and/or end-of-life programs/projects/activities.
- Priority will be given to organizations that provide services to individuals within Missoula, Lake, Sanders, and Mineral Counties.
- Priority will be given to nonprofit organizations.

Restrictions:
Program/Project Support Grants do not cover:
- Costs associated with standard clinical hospice and/or palliative care programs
- Costs which will covered by other funding sources

Projects, programs, and activities must benefit hospice patients and/or their families within Western Montana

Timeline:
- Applications Accepted: February 1, 2020 - April 30, 2020
- Application Deadline: 3:00 PM Mountain Time, April 30, 2020
- Awards will be announced no later than May 20, 2020.

There is no limit on the number of programs, projects, or activities an organization may include in their application. Each organization may submit one application per geographic location.

Administrative Requirements:
- Organizations must take the Eligibility Quiz if they wish to apply for a 2020 Program/Project Support Grant. The eligibility quiz is at the bottom of the URL for Full Text below.
- Only online applications will be accepted
- All supporting attachments must be submitted with the grant application
- Incomplete applications will not be considered for funding
- Applications received outside of the time frame set by HCF will not be reviewed

- Existing Programs/Projects/Activities: Funds will be awarded for reimbursement of incurred costs or for project costs to continue the program/project/activity
- New Programs/Projects/Activities: Funds will be awarded for projected costs
