Grants of up to $3,000 to Western Montana organizations that provide hospice and palliative care services to indigent individuals who cannot afford such care services. Priority will be given to organizations serving Missoula, Lake, Sanders, and Mineral Counties. By recognizing and acting upon this need, the Foundation strives to increase the number of individuals served.

Funding is intended to reimburse organizations for unpaid and/or unreimbursed costs of providing palliative and hospice care services, including:

-Pain & Symptom Control
-Spiritual Care
-Home Care
-Inpatient Care
-Respite Care
-Home Hospice Needs
-Patient Care Items/Services

Number of Grants:

HCF anticipates funding approximately five Compassionate Care Grants in 2019.

Estimated Size of Grant:

Maximum Awarded per Organization: $3,000.

Additional Grants or Grants of more than $3,000.00 may be awarded should additional funding become available and only if approved by the Board of Directors.

Additional Eligibility Criteria:

Eligible organizations include those in Western Montana – Flathead, Granite, Lake, Lincoln, Mineral, Missoula, Ravalli, and Sanders Counties – who provide palliative and/or hospice care services to indigent individuals.

Priority will be given to organizations that provide services to individuals within Missoula, Lake, Sanders, and Mineral Counties.

Organizations must demonstrate that they serve indigent individuals and be able to substantiate those costs which remain outstanding.

Compassionate Care Program Grants do not cover:
- Staffing or personnel costs (except in cases of bereavement services).
-Costs that will be paid by the patient, his family and/or insurance.
-Costs that will be reimbursed through other funding sources.
-Costs to be reimbursed through these grants must be otherwise outstanding after all collection efforts.
-Organizations must certify that the information submitted is accurate and true and that the expenses will not be reimbursed from other sources.

Pre-Application Information:

-Organizations must take the Eligibility Quiz if they wish to apply for a 2019 Compassionate Care Grant.
-Only online applications will be accepted.
-All supporting attachments must be submitted with the grant application.
-Incomplete applications will not be considered for funding.
-Applications must be completed and received by HCF during the specified time frame to be considered for funding.

-Applications for 2019 Grants are accepted starting December 1. 2018 and must be received by 3:00 PM MT, February 1, 2019. Applications received outside of this time frame will not be reviewed.
-Awardees will be notified no later than March 31, 2019.
