Request a Grant Writer

Grants of up to $3,000 to Western Montana organizations that provide hospice and palliative care services to indigent individuals, as well as patients who have residential hospice expenses. Priority will be given to organizations serving Missoula, Lake, Sanders, and Mineral Counties.

Grants are to reimburse organizations and/or patients for the unpaid and/or unreimbursed costs of palliative and hospice care including:

-Pain & Symptom Control
-Spiritual Care
-Home Care
-Inpatient Care
-Respite Care
-Home Hospice Needs
-Patient Care Items/Services

Estimated Size of Grant:

Maximum Awarded per Organization: $3,000.

Additional Grants or Grants of more than $3,000.00 may be awarded should additional funding become available and only if approved by the Board of Directors.

Additional Eligibility Criteria:

Eligible organizations/persons include those in Western Montana who provide palliative and/or hospice care services to indigent individuals.

Priority will be given to organizations that provide services to individuals within Missoula, Lake, Sanders, and Mineral Counties.

Organizations must demonstrate that they serve indigent individuals and be able to substantiate those costs which remain outstanding.

Compassionate Care Program Grants do not cover:
-Cost of staff salary or fringe benefits (except in cases of bereavement services).
-Costs which will be paid by the patient, his family and/or insurance.
-Costs which will be reimbursed through other funding sources.
-Costs to be reimbursed through these grants must be otherwise outstanding after all collection efforts.
-Organizations must certify that the information submitted is accurate and true and that the expenses will not be reimbursed from other sources.

Pre-Application Information:

-Organizations must take the Eligibility Quiz if they wish to apply for a 2018 Compassionate Care Grant.
-Only online applications will be accepted.
-All supporting attachments must be submitted with the grant application.
-Incomplete applications will not be considered for funding.
-Applications must be completed and received by HCF during the specified time frame to be considered for funding.
-Applications for 2018 Grants are accepted starting Wednesday, November 1, 2017 and must be received by 3:00 pm MT Monday, January 15, 2018.
-Applications received outside of this time frame will not be reviewed.
