Grants to Massachusetts, Rhode Island, and New Hampshire nonprofits and community health centers to support efforts and initiatives to improve conditions for seniors by influencing advocacy and policy to build healthy communities. Letters of Intent are due July 20, 2017. Proposals may include the areas of transportation, housing, employment, built environment, social and civic participation and inclusion, communication and information, and community support and health services.

Proposals should also include:

- Community/civic engagement of diverse stakeholders in communities to mobilize and affect change
- Participation among key stakeholders, including the voice of older adults, to support planning, policy, and advocacy initiatives
- Expanded access to programs and services as well as system change/improvement

More details on the program areas are as follows:

Community and Civic Engagement
The Foundation will support nonprofits that address or support issues/actions that lead to changes in the communities they serve. Activities can include (1) convening diverse stakeholders, (2) planning, and (3) engaging in policy activities that can lead to healthy communities and age-friendly cities.

Access to Programs and Services
Improved access to programs and services — transportation, housing, food access, etc.— by all members of a community, including older adults, will lead to a better quality of life. More information and greater awareness about how older adults can participate in improving their lives, health, and wellbeing are also necessary. The Foundation will support activities that improve access.

The grant cycle runs from January 1, 2018 to December 31, 2018. Multi-year proposals will be considered.

The Tufts Health Plan Foundation funds projects serving communities in Massachusetts, New Hampshire, and Rhode Island. The Foundation supports community-based organizations and community health centers; support to hospitals or medical centers will only be considered if project includes community-based partners.

The Foundation does not fund:

- Candidates for public office
- Capital projects and/or equipment
- Development activities, such as fundraising events, capital campaigns, or annual fund drives
- Endowments
- General operating expenses
- Government agencies (unless within a collaboration led by a community-based organization)
- Individuals (including scholarships, internships, and/or fellowships)
- Lobbying
- Organizations that advocate, support or practice discrimination based on race, religion, age, national origin, language, gender, gender preference, or physical handicap
- Private foundations
- Religious organizations for religious purposes
- Research for specific disease states
- Underwriting of medical expenses

Full Proposals Due: September 18, 2017
