Request a Grant Writer

Grants to Colorado nonprofit organizations for operational support and programming that promotes health and wellbeing for Western Slope residents. Proposals may focus on physical health, behavioral health, and the social determinants of health.

Specifically, grants will be awarded to organizations and programs that address:

- Access to healthcare, including telehealth
- Preventive care and services
- Behavioral health services
- Case management
- Population health (including integrating social service programs and health systems)
- Community-based projects

To be considered for a grant, organizations must:

- Address a Community Health Needs Assessment gap and make a measurable and positive impact on the health of the specific community. See the link in Pre-Application Information for more details.
- Focus on at least one of the following focus areas listed above.
- Accomplish clearly-defined goals as outlined in the grant application.
- Have a clear strategy to implement and manage the proposed project.
- Demonstrate fiscal responsibility through a detailed and realistic project budget that itemizes expenses and projected revenue.

Term of Contract:

Rocky Mountain Health Foundation will consider one-year funding commitments. The funding period for this cycle is July 2, 2018 - June 28, 2019.

Additional Eligibility Criteria:

Applicants must answer "yes" to all the following questions to be eligible.

- Are you a 501 (c)(3) or County Public Health Department? If not, do you have a fiscal sponsor?
- Are you in good standing with the Colorado Secretary of State?
- Do you serve in one of the 22 western slope counties as defined by RMHF’s service area? The service area is defined as the following Counties: Archuleta, Delta, Dolores, Eagle, Garfield, Grand, Gunnison, Hinsdale, Jackson, La Plata, Lake, Mesa, Moffat, Montezuma, Montrose, Ouray, Pitkin, Rio Blanco, Routt, San Juan, San Miguel, and Summit.
- Does your project funding request align with the priorities identified by your hospital or Public Health Dept’s Community Health Needs Assessment? See the link in Pre-Application Information for more details.
- Have you discussed your funding request with representatives from your local public health agency, and ensured that your project will advance the overall health and well-being of the community?

Grants are awarded quarterly. All applications must be submitted online no later than 5:00 PM by the due dates.

1st Quarter Timeline:
- Application Open: December 22, 2017
- Application Due: January 24, 2018
- Award Notification: March 15, 2018
- Funding Period Begins (Funds Released): April 2, 2018
- Funding Period Ends: March 29, 2019
- Final Report Due: April 26, 2019

2nd Quarter Timeline:

- Award Notification: June 14, 2018
- Funding Period Begins (Funds Released): July 2, 2018
