Grants to Iowa local health boards to provide cancer screenings, patient care, outreach, and diagnostic services for women in eligible counties. Grant recipients will identify a local health agency or organization to provide local services to eligible women.
Services covered by this application include financial support to local boards of health to assure the provision of outreach, program enrollment, breast and cervical cancer screenings and diagnostic services, data reporting, and patient navigation services for eligible persons.
IDPH currently contracts with 10 local boards of health (identified eligible applicants for this application) for enrollment, referral, cardiovascular intervention delivery, data collection and recall of eligible women. Women from 52 Iowa counties are eligible for enrollment, and receive program services through the 10 contracted local boards of health which identify a local public health agency or health organization to perform the local administration.
Contractors are responsible for providing cardiovascular screening and referral for abnormal screening results to low-income, underinsured or uninsured participants of the Care for Yourself Breast and Cervical Cancer Early Detection Program.
Additionally, contractors provide risk reduction counseling (results of lipid and glucose panels and other cardiovascular risk factors) to all women enrolled in the program. Contractors provide women with the option to participate in health coaching, Weight Watchers, Blood Pressure Self-Monitoring, Medication Therapy Management, Diabetes Prevention Program, Diabetes Self Management Education Support and the YMCA’s Blood Pressure Self-Monitoring Program. Further, contractors refer women to community-based resources.
Contractors assist the eligible individuals with obtaining the recommended or required services by working with approved healthcare providers. Approved health care providers then receive reimbursements through a third-party for provision of the approved screening and diagnostic services based on approved CPT codes paid at Medicare Part B Participating Provider Rates.

The Department expects the contract period to be a 10 month term from November 30, 2018 to September 29, 2019.
Local Boards of Health (LBOH) are listed in the FY19 Funding Allocation Table attached below in Supporting Documents, and will be the only entity eligible to apply for and serve as the contractor for the FY19 Care for Yourself (CFY) - WISEWOMAN Program contract. Local Boards of Health (LBOH) have jurisdiction over public health matters in their designated geographic area.
A Local Public Health Agency may not apply in response to this posting.
