Grants to Colorado private dentists, community organizations, health centers, clinics, and health agencies for dental services for low-income adults over age 60. Funding is intended to provide dental care to senior citizens who do not have other means to receive this care (i.e. do not have dental insurance coverage and cannot afford to pay for private dental care).

Qualified Grantees are required to:
- Identify and outreach to targeted Eligible Seniors and Qualified Providers;
- Demonstrate collaboration with community organizations;
- Ensure that Eligible Seniors receive Covered Dental Care Services efficiently without duplication of services;
- Maintain records of Eligible Seniors served, Covered Dental Care Services provided, and monies spent for a minimum of six (6) years;
- Distribute grant funds to Qualified Providers in their service area or directly provided Covered Dental Care Services to Eligible Seniors in their service area; and
- Expend no more than seven percent (7%) of the grant amount for administrative purposes.

Potential grant applicants located in the following counties are encouraged to apply: Archuleta, Baca, Cheyenne, Clear Creek, Crowley, Custer, Delta, Dolores, Eagle, Garfield, Gilpin, Grand, Gunnison, Hinsdale, Huerfano, Jackson, Kiowa, La Plata, Las Animas, Mineral, Moffat, Montezuma, Ouray, Park, Pitkin, Prowers, Rio Blanco, Routt, Saguache, San Juan, and San Miguel.

Term of Contract:

The initial contract term resulting from grant awards is one year: July 1 through June 30. Subject to available funding and contractor performance, the Department may renew the contract annually for up to three (3) additional years.

Additional Eligibility Criteria:

Area Agencies on Aging, community-based organizations or foundations, Federally Qualified Health Centers, safety-net clinics, health districts, local public health agencies, and private dental practices are encouraged to apply.
