Grants of up to $50,000 to North Carolina nonprofit organizations and government entities in eligible locations to address social health determinants. Funding is intended to support organizations that engage in leverage-building collaborations and offer innovative ideas for local regions. Eligible programs strengthen the overall health of communities in Cherokee, Clay, Graham, Jackson, Macon, Swain counties and the Qualla Boundary. Social determinants of health include factors such as housing, education, transportation, access to quality foods, and social support networks.
Recipients of the Collaborative Health Innovation Project, or CHIP grants, will demonstrate how their problem-solving project improves health outcomes for everyone. They will work inclusively with their communities, make the most of existing resources, and collaborate with at least one other organization as part of their problem-solving and/or response process.
