A hospital integrated postpartum peer support program offered to families before they leave the hospital with their baby. Families receive a full year of connection through personal outreach, community groups, educational opportunities, family events, expert resources, and access to additional YANA services.
Peer Support Groups
Free, welcoming groups where moms can connect with others who understand the realities of motherhood. Groups provide a safe space for honest conversations, encouragement, friendship, and support without judgment.
Support Mom Program
Moms are matched with a trained volunteer Support Mom who offers consistent, personal encouragement based on shared experiences. A Support Mom is not a therapist or medical provider. She is a trusted peer who listens, checks in, and reminds another mom that she does not have to navigate motherhood alone.
YANA Community Room
A free resource space where families can access donated baby supplies, maternity items, clothing, diapers, hygiene products, and other essentials. Families are welcomed with dignity and can take the items they genuinely need.
Let’s Be Real Mama Journeys Podcast
A podcast featuring honest conversations about motherhood, mental health, caregiving, grief, resilience, relationships, and lived experience. Each episode helps normalize difficult conversations and reminds listeners that real motherhood does not have to look perfect.

- YANA’s primary target audience is moms during the first year after having a baby, regardless of income, background, diagnosis, or previous parenting experience. Through Strong Mamas, Thriving Babies, support is offered universally to families leaving participating hospitals with a live baby.
YANA also supports moms throughout every stage of motherhood through peer support groups, Support Mom connections, community events, educational resources, the Community Room, and the Let’s Be Real Mama Journeys podcast.
YANA places particular emphasis on moms who may experience increased isolation or barriers to support, including single moms, teen moms, moms of color, moms in recovery, moms experiencing postpartum mental health challenges, families with limited resources, and adoptive or kinship caregivers. Strong Mamas, Thriving Babies
Funding for hospital coordinators, bedside enrollment, yearlong family outreach, support groups, educational programming, technology, data tracking, and community events.
Program Expansion
Startup funding for new hospital partnerships, including staff recruitment, training, hospital credentialing, outreach materials, technology, and local community engagement.
Staffing
Support for program directors, hospital coordinators, peer support staff, volunteer coordination, development, data management, and administrative operations.
Community Room
Funding for diapers, wipes, hygiene products, baby supplies, maternity items, storage, and the costs of maintaining a free and welcoming resource space.
Peer Support Services
Resources to recruit, train, supervise, and retain Support Moms and group facilitators who provide consistent support to families.
Transportation and Accessibility
Funding to help families overcome transportation, language, technology, and other barriers that may prevent them from accessing services.
