Target Audience
Many in our community face significant barriers to healthcare, often resorting to 911 calls and hospital visits as their only means of accessing care. These individuals are also challenging to discharge from acute care settings due to limited support systems. At Care Bridge, we bridge these gaps, delivering essential care and support to those who need it most, right where they are.
