- TEAM ECHO (Eliminating Community Health Obstacles) is a Georgia-based 501(c)(3) nonprofit founded in 2021 to reduce health disparities through community-based prevention and outreach. We provide free preventive health screenings, health education, chronic disease prevention, care navigation, and resource connection for uninsured, underinsured, elderly, homeless, and other underserved populations. Our outreach takes place in trusted community settings where we meet people where they are and connect them with resources that improve long-term health and well-being. Since launching our outreach efforts, we have screened more than 1,000 community members and continue to expand our impact through partnerships with local organizations.

- Our goal is to become a leading community health nonprofit serving Coastal Georgia by expanding access to preventive healthcare, reducing health disparities, and improving health outcomes for underserved communities. Over the next three to five years, we plan to increase the number of individuals served through community outreach, establish sustainable partnerships with healthcare systems and community organizations, secure long-term grant funding, and develop innovative programs that address both health needs and the social factors that influence overall well-being. We are committed to building a sustainable organization that creates measurable, lasting impact across the communities we serve.
