Grants of $100,000 to improve the quality of child and adolescent obesity health care. Funding will be provided to organizations to recruit and support local health care provider organizations and primary care practices in order to ensure recommended practices for child and adolescent obesity screening. Funds will be provided for outreach, technical assistance, training, quality improvement monitoring and other support needed to achieve and sustain improvements in health care systems and provider practices in accordance with the Expert Committee Recommendations. The goal is to enhance the delivery and quality of assessment, preventive counseling, and office based follow-up practices.

Additional Eligibility Criteria:

Eligible applicants include local government agencies and nonprofit organizations such as: educational institutions, hospitals, insurers, professional organizations, local health departments and other nonprofit agencies in NYS with experience and expertise in the administration of health care delivery systems and in the provision of training and technical assistance to clinical staff.

To be eligible to apply, an applicant must be an established organization located within the catchment area to be served: Bronx, Kings, New York, Richmond, Queens, Nassau, Suffolk, Orange, Putnam, Rockland, Westchester
Dutchess, Sullivan, Ulster, Clinton, Essex, Franklin, Hamilton, Fulton, Montgomery, Saratoga, Warren, Washington
Albany, Columbia, Delaware, Greene, Otsego, Rensselaer, Schenectady, Schoharie, Broome, Chenango, Cortland, Tioga, Tompkins, Herkimer, Madison, Oneida, Cayuga, Onondaga, Oswego, Jefferson, Lewis, St. Lawrence, Allegany, Cattaraugus, Chautauqua, Wyoming, Erie, Genesee, Niagara, Orleans, Monroe, Livingston, Ontario, Seneca, Wayne
Chemung, Schuyler, Steuben, Yates
