Grants ranging up to $250,000 to support wellness, health, and access to high quality health care through initiatives that improve health and health care systems. Funding is directed to organizations that address the following health issues: obesity among children and adults, racial and ethnic health care, and integrated health care. Integrated health care should improve care coordination and communication among health care providers, create informed and involved patients, and promote cost-effective and affordable care. Support will be awarded for research, project, and/or policy grants.
Grants may also be awarded for sponsorship of fundraising events, galas, walks, outreach activities and other community-based health and wellness initiatives.

Grants will be made only to nonprofit organizations with evidence of IRS 501[c](3) designation or de facto tax-exempt status.
National grant proposals must demonstrate results nationwide and may not be limited to one geographic area.
Regional grant proposals must be concentrated on a specific region, state or community from the eligible areas listed below: Phoenix, Arizona; Los Angeles, California; Connecticut; Miami, Florida; Atlanta, Georgia; Chicago, Illinois; Baltimore and DC areas, Maryland; New Jersey; NYC, NY; Philadelphia, Pennsylvania; Dallas, Houston, Austin, and San Antonio, Texas.
Letters of Inquiry are accepted on a rolling basis. Full proposal application deadlines will be provided with notification that a Letter of Inquiry has been accepted.
Full proposals for requests up to $50,000 will be accepted without preliminary submission of a Letter of Inquiry. Applicants may be required to submit additional information during the review to meet the application guidelines for their type of project.
