Grants to Utah public, community-based, for-profit, and nonprofit primary healthcare providers for costs to provide basic medical care to uninsured and underserved populations. Projects may address dental, mental health, or medical services in local communities.

Program objectives for funded projects are to:

- Improve the health of eligible individuals.
- Provide continuity of care to eligible individuals who receive ambulatory primary care services.
- Increase the number of eligible individuals served in the target populations (working poor, children, elderly, Native Americans, migrant and seasonal farmworkers, individuals with chronic diseases, the homeless, individuals with limited English-speaking proficiency, and single heads of household).
- Increase the number of eligible encounters by individuals in the target populations.
- Increase the organizational capacity of the grantee to provide ambulatory primary health care services to eligible individuals and their families by increasing ongoing and long-term capacity.

Estimated Size of Grant:

Agencies may submit one application per site for up to $100,000 per site.

The grant period will cover July 1, 2019 through June 30, 2020.

Additional Eligibility Criteria:

To be eligible, your agency must provide primary medical care services, OB/GYN care, dental services,mental health services, or other professional services. If your agency plans to use State Primary Care GrantProgram funding to provide only enabling services, your application will be denied.

All public entities, community-based organizations, and private entities (including for-profit and nonprofit,as defined in Utah Code ยง 26-10b) are eligible to apply.


- SPCGP funding cannot be used for research.
- Funding from the SPCGP cannot be used specifically for health education or education classes. Health education is covered at the time of the primary medical care, dental, and mental health client visits only.
- The SPCGP does not cover inpatient substance abuse treatment.
- Funding from the SPCGP cannot be used for staff travel or transportation costs. However, travel expenditures may be granted to mobile clinics with a reasonable justification and explanation of costs.
- Funding from the SPCGP can only be used for legal residents of Utah, or for military personnel or their spouses who are in Utah on military assignment. A legal resident of Utah is defined in the Complete Final Package (see Supporting Documents below).
- SPCGP funding cannot be used to purchase equipment.
- Any pharmaceutical costs are considered part of the charge per encounter.

Pre-Application Information:

A complete, original, electronic application must be submitted to the Office by Monday, May 13, 2019 5:00 p.m. M.S.T.

OPCRH expects to announce grant awards in June 2019.

Agencies may submit one application per site for up to $100,000 per site. The OPCRH will not accept more than one application per site. Agencies that propose multiple projects for a single site must include all projects for that site in a single application. Agencies that have previously submitted multiple projects must either 1) propose a single project or 2) combine multiple projects into a single application.
