One grant of up to $98,000 to a New York City qualified consulting firm or consultant to assist the successful involvement of the funding source and its network of providers in healthcare reform initiatives as it relates to the aging population. The consultant will attend various planning and development meetings on the funding source's behalf, and create and execute a plan for the enhancement and development of the programmatic and administrative aspects of funding source health programs.
While progress has been made in developing relationships and fully understanding the nuances of Medicaid reform in New York, more needs to be done in order for Department for the Aging (DFTA) to be fully recognized as a partner and resource for Medicaid enrollees, health plans and performing provider systems.

The contract amount shall not exceed $98,000.
The anticipated contract term shall be for a period of six months approximately from October 13, 2014 to March 31, 2015.
