Grants to Minnesota nonprofit organizations and governmental or tribal entities to provide clinical health service programs for American Indians living off Indian reservations. Funding is intended to support stationary or mobile clinics and eligible programming includes medical, dental, preventative, or mental health care.

“Clinical health services” means outpatient or ambulatory services for the diagnosis or treatment of illness or injury or the maintenance of health. Clinics may provide one or more of these services and are not required to provide all.

Proposed projects must result in a direct and quantifiable patient benefit for this population.

Term of Contract:

Funding is provided for two years, January 1, 2020 – December 31, 2021. It is expected that applicants will be able to complete the proposed project during the grant period.

Additional Eligibility Criteria:

Eligible entities include nonprofit organizations, governmental or tribal entities providing clinical health services to American Indians who reside off reservations.

For non-tribal entities, collaboration with the American Indian patient population or established organizations representing the American Indian population is required.

Ineligible expenses include but are not limited to:
- Fundraising
- Taxes, except sales tax on goods and services
- Lobbyists, political contributions
- Bad debts, late payment fees, finance charges, or contingency funds

Clinics must not be solely for the purpose of urgent nor emergent care.

Pre-Application Information:

Applications must be received by 3:00 PM on October 4 Central Time. Late applications will not be accepted.

Please submit questions no later than 12:00 PM Central Time, on September 20, 2019.

Applicants will be notified of award decisions by U.S. Postal mail and email, if possible, in November 2019.
