Grant Description
Grant of up to $20,000 to a New Hampshire agency, nonprofit, business, or individual to initiate a physical activity program Statewide targeting licensed child care programs, and provide physical activity consultations. Applicant must be able to travel throughout the State and have a bachelor's degree or certification in physical education or early childhood education, or the equivalent.

The New Hampshire Department of Health and Human Services (DHHS), Division of Public Health Services, (DPHS), Bureau of Population Health and Community Services (BPHCS) is accepting applications from vendors to provide physical activity consultation and implement a statewide physical activity project focused on Licensed Child Care Programs.

The selected vendor will:

- Work with Department staff to identify and solicit interest from at least seven (7) licensed child care programs that will participate in the project.

- Will provide qualified staff to assist the Licensed Child Care Programs in completing two physical activity assessments included in the Nutrition and Physical Activity Self-Assessment for Child Care ( Go NAP SACC), http://www.gonapsacc.org, program.

-- The Go NAP SACC program is child care health consultation and training designed specifically for use in child care settings.

--The NAP SACC program promotes healthy weight development in preschool children by helping child care programs improve.

Additional Eligibility Criteria
Staff charged with services/items/responsibilities related to this contact must meet the following requirements:

Education and Experience:
-A Bachelor’s degree/certification in physical education or its equivalent, or in early childhood education or its equivalent; Applicant must provide transcripts.
- Have prior work experience with child care programs preferably focusing on physical activity; and
- Must have experience and/or knowledge of working with the Go NAP SACC program.
- Provide references.
- Provide an update resume(s).
- License/Certification: Valid driver’s license and/or access to reliable transportation with liability insurance coverage as required by state laws for travel throughout the state.
- General Liability Insurance.
- Have the ability to become a vendor with the State of New Hampshire, http://das.nh.gov/purchasing/vendor.asp.

Applicant(s) must not be presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from covered transactions by any Federal department or agency.

Estimated Size of Grant
-The rate of compensation for State Fiscal Year 2016 is an all-inclusive rate of $50 per hour for services up to 10 hours per week, not to exceed 240 hours per year for an amount of $12,000 per year.

- The rate of compensation for sub-awards is up to $1,000 per child care program for a maximum amount of $8,000 for sub-awards.

-The maximum total contract value is $20,000 per year for the Physical Activity Consultation in Child Care Project.

Term of Contract
Services will begin upon the date of Governor and Executive Council approval through June 30, 2016.

The Department reserves the right to renew contracts for up to two (2) additional years, subject to continued availability of funds, satisfactory performance of services, and approval by the Governor and Executive Council.
