Grants ranging from $1,000 to $15,000 to Douglas County, Kansas nonprofit organizations or government agencies to promote healthy eating and physically active lifestyles for community residents. Priority will be given to programs that focus on children's health. Projects should help the community make progress toward achieving goals set forth in the 2015 Douglas County Community Health Plan.

LiveWell Community Wellness Grants seek to support lasting measures that improve residents’ health. For this reason, favored projects are those that create long-term policy, systems, and environmental change. Funding programs that increase access to healthy food and/or promote healthy eating and physically active lifestyles, especially if the program advances a larger community goal, will also be considered.

Grants will help Douglas County organizations:
-Adopt policy, systems, and environmental changes that support healthy eating and physical activity in everyday environments such as childcare centers, schools, after-school programs, workplaces, community centers, neighborhoods, and places of worship
-Offer programs that help residents make lasting improvements to their diets and activity patterns that reduce their risk for lifestyle-related chronic diseases such as hypertension, heart disease, diabetes and cancer.

Grants will also foster collaboration and a sense of shared purpose among organizations committed to working together to improve community health.

Favored projects:
-Provide a substantial benefit over time
-Produce meaningful, measurable results
-Can be sustained by organizations without ongoing LiveWell Community Wellness Grant funding
-Help attract additional funds and resources for the participants or organization(s)
-Assist with or promote collaboration among nonprofits, schools, employers, neighborhoods, community centers, tribal entities, and/or local governments

In addition, the Foundation is more likely to fund proposals that:
-Are clearly and concisely written
-Include a workable plan to measure results
-Are based on complete and clear income and expense projections
-Are proposed by organizations that demonstrate sound financial practices and strong staff and volunteer support
