Grant Description

Grants of $15,000 to California nonprofit organizations and school-based health programs to improve the health of local residents in the funding source's service area. Funding will be awarded to programs and projects in the areas of health equity and wellness. Collaborative requests, as well as requests for general operating support, will be considered. Requests should serve individuals living in the Community Health Trust's primary service area in the Pajaro Valley.

Requests must align with one or more of the Health Trust's strategic focus areas: Building Health Equity: An equal and fair opportunity for every individual to attain the highest level of health and wellness regardless of socioeconomic barriers, and/or Increasing Wellness in Pajaro Valley: The active pursuit of activities, choices, and lifestyles that emphasize preventing illness and improving quality of life, as opposed to emphasizing disease treatment.

Eligibility:

Additional Eligibility Criteria:

Applicant must be a nonprofit, 501(c)3 tax-exempt organization; a school-based health program; or have a 501(c)3, tax-exempt organization as a fiscal sponsor.

CHT’s Board prefers not to fund programs or projects administered by government agencies, with the exception of school-based health programs closely aligned with CHT’s mission.

Ineligible:

The Health Trust does not fund:
- Individuals, with the exception of the Health Trust’s scholarship programs.
- Religious organizations for purposes of evangelizing, proselytizing, or converting.
- Endowments, building campaigns, annual fund appeals, fundraising events, or celebrations.
- Commercial ventures.
- Previously-completed activities.
- Debt reduction.

Pre-Application Information:

The Trust will accept applications for the 2023-2024 grant cycle beginning November 11, 2022 – January 20, 2023.

While there is no formal policy about the number of consecutive years an organization is eligible to receive support from the Health Trust, grantees are typically invited to take a year’s hiatus from applying after four consecutive grants.
