Grants to Massachusetts, New Hampshire, Rhode Island, and Connecticut nonprofit organizations for new or established programs that strengthen community inclusion for older adults. Applicants must submit a Letter of Intent prior to submitting a full proposal. Funding is intended to support activities that lead to age-friendly communities and address barriers to success.

The Foundation seeks to fund efforts that convene, support, and expand collaborations that lead to systems/service access and improvement on a larger geographic scale (multiple counties, statewide, or New England-wide) or across multiple sectors.

Additional Eligibility Criteria:

The Tufts Health Plan Foundation supports the work of nonprofit organizations serving communities in Massachusetts, New Hampshire, Rhode Island, and Connecticut.

Not Funded:
- Candidates for public office
- Capital projects and/or equipment
- Development activities, such as fundraising events, capital campaigns, or annual fund drives
- Endowments
- General operating expenses
- Government agencies (unless within a collaboration led by a community-based organization)
- Individuals (including scholarships, internships, and/or fellowships)
- Lobbying
- Organizations that advocate, support or practice discrimination based on race, religion, age, national origin, language, sex, sexual preference, or physical handicap
- Private foundations
- Religious organizations for religious purposes
- Research for specific disease states
- Underwriting of medical expenses

- Letters of Intent Due: Friday, January 22, 2021
- Notification of Invitation: Friday, February 26, 2021
- Full Proposals Due: Friday, April 2, 2021
- Notification of Award: Friday, June 25, 2021
- Grant Period: July 1 - June 30

Applications are due by 4 p.m. ET on the dates specified.

Organizations may submit multiple letters of intent (LOI), but only one grant will be considered per year. Only one grant will be awarded per organization during any given cycle.
