Grants of $10,000 to Missouri nonprofits for programs that will enhance the health of residents in eligible communities. The goal of the Foundation will be to improve the health of local residents by addressing mental health issues, improving access to health care for underserved and under-insured populations, and reducing the rate of chronic disease. Eligible communities include the counties of Clay, Platte, Ray, and Buchanan.

Current priorities are:

Access to Care
- Increased access and improved quality of life for under-insured and underserved populations, including those living at or below the poverty line, minorities, single-parent households, rural communities, and those with developmental or physical limitations.
- Providing pediatric services from newborn to age 18, including but not limited to well-child care, preventative medicine, physical examinations, immunization, oral health, vision, speech and hearing, and injury prevention.

Chronic Disease
- Reducing rates and/or prevention of chronic conditions, including cancer, heart disease, COPD, diabetes, stroke, and obesity
- Focus on nutrition and physical activity.

Mental Health
- Substance abuse resources and support
- Mental health care access
- Drug and alcohol education and tobacco prevention

The grant is for a one-year period.

Additional Eligibility Criteria:

Eligible applicants must have active and documented 501(c)(3) nonprofit status.

Core services must be delivered within the service area of Liberty Hospital, which includes Clay, Platte, Ray, and Buchanan counties in Missouri.

Special consideration will be given to agencies that can demonstrate their participation in unique community partnerships and collaborations. Agencies with new and innovative programs are encouraged to apply.

Pre-Application Information:

All grant applications must be received by noon on February 8, 2019. All applicants will be notified of their award status by February 28, 2019.

Awardees will be recognized at the Liberty Hospital Half Marathon on March 2, 2019, and grants will be distributed at a post-event ceremony later that month.
