Grants to Kansas and Missouri nonprofit organizations for programs to improve the health of local residents in the funding source's service area. Applicants are required to submit an LOI prior to applying. Areas of focus include disease detection and prevention, enhancement of health care services, and addressing factors affecting the health of men of color. Projects must target people who live in the seven-county area served by PHF: Jackson, Clay, Platte, Cass, and Ray counties in Missouri and Johnson and Wyandotte in Kansas.

The Prime Health Foundation funds projects that meet one of the three Focus Areas:

  1. Prevention or early detection of diseases.
  2. Improvements in the quality, availability, and/or accessibility of health care services.
  3. Improvements in conditions that lead to, or outcomes that are the result of, health care disparities affecting young men of color, ages approximately 14 – 44.

In addition to falling within one of the three Focus Areas, all projects must:

  • Address a critical need in the community
  • Demonstrate culturally responsive practices
  • Target persons who are poor, uninsured, and/or underinsured

Estimated Size of Grant:

The average amount funded to grantees is approximately $16,000. Grantees requesting amounts significantly greater are likely to have their request reduced by the Board of Directors.

Pre-proposal Conference:

Interested applicants are invited to participate in an optional Zoom Q&A session with Prime Health Foundation officials on April 7, 2021 at 12:00 p.m. Central Time. It should be noted there will be no formal presentation, and those planning to attend are encouraged to review the Prime Health Foundation website in advance. Registration is required. Pre-Application Information:

- Letters of Intent (“LOIs”) for 2022 funding are due by 5:00 p.m. on Friday, May 28, 2021.
- Organizations submitting LOIs will be notified whether they are invited to submit full applications by Tuesday, June 15, 2021.
- For those selected to apply, the due date for online application submissions is 5:00 p.m., Friday, July 30, 2021.
- Notifications of funding decisions will be made by email in the December 2021/January 2022 timeframe. In January 2022, checks for the 1st half of the year will be released, and Grant Agreement Letters must be signed within 14 days.

Organizations that would like to apply for funding must complete and submit a Letter of Intent. Based on the number, type and characteristics of organizations and programs from which Letters of Intent are received, selected organizations will be invited to complete full applications.
