Grant of up to $60,000 per year to an Iowa nonprofit, for-profit, local health board, public health agency, or government agency to collect data on risky behaviors among youth. The purpose of this program is to build knowledge in order to address health inequities within the state.
The purpose of this Request for Proposal (RFP) #58819014 is to solicit applications that will enable the Iowa Department of Public Health (referred to as Department) to select the most qualified applicant to provide data collection services for the Iowa Youth Risk Behavior Survey (YRBS) and School Health Profiles (collectively the Iowa Youth Risk Behavior Surveillance System (YRBSS)).
The collection of data on Iowa youth risk behaviors actively works to strengthen the Department’s capacity to address health inequities in Iowa. Health equity is defined as the attainment of the highest possible level of health for all people by achieving the environmental, social, economic, and other conditions in which all people have the opportunity to attain their highest level of health. This RFP promotes health equity through collecting important information to understand disparities in health outcomes and risk behaviors among youth.
Goals and Objectives of the program:
- Goal 1: Increase the understanding of youth risk behaviors in Iowa.
- Objective 1: Administer the YRBS and achieve a 60% response rate of sampled students.
- Goal 2: Increase the understanding of school health policies and practices in Iowa.
- Objective 2: Administer School Health Profiles and achieve a 70% response rate of sampled principals and health teachers.

The Department funding will be available for approximately one award.
Applicants may apply for up to $60,000 for the total 10 month period. The Department anticipates up to $60,000 available for approximately one award. The Department anticipates up to $60,000 to be available for each of the remaining four years. Actual total awards and individual contract funding levels may vary from that listed or funding may be withdrawn completely, depending on availability of funding or any other grounds determined by the Department to be in the Department’s best interests.
The project period shall be from 10/1/2018 to 7/31/2023.
The Department expects the initial contract period to be a ten month term from 10/1/2018 to 7/31/2019. The Department shall have the option to renew the contract at its sole discretion for up to four additional one (1) year terms subject to review of the continuation application, contractor performance and compliance with the terms and conditions of the contract, availability of funds, program modifications, or any other grounds determined by the Department to be in Department’s best interests.
Nonprofit and for-profit organizations, Local Boards of Health, public health agencies, and governmental entities are eligible to submit an application in accordance with this RFP.
